Lots of ways to do it pretty simply on your own. Try running 'reply form script' through Google (or your favorite search engine), and I"m sure you'll find a selection depending on the language you want to use. You may even be able to get away with doing a basic POST and a 'mailto:your@name.com' in the ACTION. If you get stuck, PM or email and I'll try to help.
