VOiP and interface with Outlook 2003

Is there a way to interface Outlook 2003 to VOIP? I note that Nortel announced a multimedia plug-in for Outlook 2003 that interfaces with their server systems.

That's a corporate solution, but I wonder if there are solutions for small businesses. Anybody up to date on this issue? It's a hot issue, but maybe it's early days still. The field is exploding. I don't use the landline anymore for long distance calls--only VOIP.
